This document is a template notice of assignment.

This precedent should be used as the template for formal notice required for the statutory assignment of a debt or benefit of a contract. However, if used for the assignment of a benefit under a contract, the template should be adapted accordingly.

A notice of assignment may also be used, if adapted, for statutory assignments of other choses in action and for the assignment of equitable choses in act.
